Social support for older people

Holdsworth helps you stay active, connected and safe at home and in the community. The Commonwealth Home Support Program (CHSP) provides government-subsidised, entry-level support for people aged 65 and over, or 50 and over for Aboriginal and Torres Strait Islander people, who need help with everyday tasks and social connection.
 
In-home aged care is changing, and CHSP is expected to transition to Support at Home no earlier than 1 July 2027. In the meantime, CHSP remains available for eligible clients.

Allied Health & Therapy

Our Allied Health offering currently focuses solely on our Wellbeing Group, supporting older adults to maintain mobility and confidence at home and in the community. Delivered through approved CHSP services, the group is designed to promote wellbeing, safe movement and social connection in a supportive environment.

Domestic Assistance

We know how important it is to maintain a clean, comfortable home. Services in our Domestic Assistance program help maintain a clean and comfortable home. These include light cleaning, laundry support and household tasks to make daily living easier and more manageable.

Transport

Our Transport service supports independence and mobility, helping people get to appointments, social activities and essential errands safely. We provide door-to-door transport with friendly, reliable support across the Eastern Suburbs.

Meal Delivery

Enjoy tasty affordable meals delivered direct to your door. Meals are prepared and delivered by Lite n' Easy and ready to heat.

Individual Social Support

One-to-one social support provides personalised companionship and assistance. We help people stay connected through social outings and maintain meaningful routines with the support of a friendly team member or volunteer.

Group Social Support

Our Group Social Support services offer exciting opportunities to explore the community and participate in recreational activities. We provide transport and experienced staff support, helping participants feel comfortable and supported throughout each activity.

Who is eligible for CHSP?

Commonwealth Home Support Program eligibility is for people who need some assistance with daily tasks but can generally manage on their own.

Eligibility generally includes:

  • People aged 65 years or older, or 50 years or older for Aboriginal and Torres Strait Islander people
  • People who need help with specific activities rather than ongoing complex care
  • Completion of a Regional Assessment Service assessment through My Aged Care

This eligibility pathway applies to the aged care Commonwealth Home Support Program and is different from ACAT assessments used for higher levels of care or Support at Home.

CHSP vs. Support at Home program

The Commonwealth Home Support Program provides entry-level support, while the Support at Home Program is designed for people with higher or more complex care needs.

In simple terms:

  • CHSP supports specific services and social connections.
  • Support at Home provide coordinated, higher-level care.

CHSP can be a starting point before accessing broader aged care at home services.

Applying for the Commonwealth Home Support Program (CHSP)

Step 1: Contact My Aged Care

Contact My Aged Care to request an assessment for CHSP services. You’ll also be guided through the Commonwealth Home Support Program fees, which are generally low and based on contribution rather than full cost.

Step 2: Complete an aged care assessment

An aged care assessor will discuss your daily routines, needs and goals to determine your eligibility and confirm which Commonwealth Home Support Program services may be suitable.

Step 3: Choose Holdsworth as your provider

Once approved, you can select Holdsworth from the list of CHSP providers to deliver your services.

Step 4: Start your services

Services begin after assessment, with support delivered by Commonwealth Home Support Program providers to suit your lifestyle and preferences.
