Through the Commonwealth Home Support Program (CHSP), Holdsworth offers Group Social Support services that connect older people with their community through outings, activities and cultural events that bring joy and connection.

What is Group Social Support under CHSP?

Group Social Support is a funded service type under the Commonwealth Home Support Program (CHSP). It brings older people together in a group setting to take part in social activities, outings and experiences with the support of the Holdsworth team members.

These activities give you a chance to get out, meet new people, try new experiences, and stay engaged with your community.

What does group Social Support include?

Holdsworth's Group Social Support activities vary and evolve to reflect what our participants enjoy and value. Activities can include:

  • Community outings to parks, museums, gardens, restaurants, and local landmarks
  • Cultural events celebrating the diverse backgrounds of our community
  • Arts and crafts sessions and gentle exercise groups
  • Social lunches and morning teas
  • Seasonal events and celebrations
  • Visits to community venues and places of interest

Who is eligible for Group Social Support through CHSP?

To access Holdsworth's CHSP-funded Group Social Support, you need to:

  • Be aged 65 or over (or 50 or over for Aboriginal and Torres Strait Islander people)
  • Have a current My Aged Care assessment that includes Group Social Support referral code
  • Live in an area where Holdsworth delivers services

  • If you are not yet registered with My Aged Care, call 1800 200 422 or visit myagedcare.gov.au. Assessment is free.

    Why social connection matters for your health

     

    Research from the Australian Institute of Health and Welfare shows that social isolation is associated with higher rates of depression, cognitive decline and poorer overall health outcomes in older adults. 

     

    On the other hand, regular social engagement has been shown to support cognitive health, reduce rates of depression, and improve physical wellbeing. But beyond the research, most people simply know it to be true. Life is better when you feel connected. 

    How to access Group Social support through Holdsworth

    • Step 1: Register with My Aged Care by calling 1800 200 422 or visiting myagedcare.gov.au.
    • Step 2: Undergo a free assessment to confirm eligibility for CHSP and identify which services suit your needs.
    • Step 3: Choose Holdsworth as your provider for Group Social Support. We will help you find the right activities in your local area.
      Transport to and from Group Social Support activities is available. Ask us about our Transport service when you make contact.
