What is the Support at Home Program?
The Support at Home Program is a new Australian Government initiative that, on 1 November 2025, replaced Home Care Packages to create a simpler and fairer way to fund and deliver aged care services at home.
As an Approved Provider, Holdsworth delivers quality, government-funded care to help older adults stay independent and connected at home, including:
- Personal care
- Transport
- Nursing
- Domestic assistance
- Social support

New applicants
If you are applying for the first time, you can access Support at Home through My Aged Care on 1800 200 422. Following an assessment, you will be assigned a classification that aligns with your care needs and a quarterly budget to spend on approved services. Your plan is designed around your circumstances and has the flexibility to change as your needs evolve.
Pricing
Your cost depends on your assessed classification, your income and assets (in some cases), and the types of services you choose. At Holdsworth we are transparent with our pricing: we publish standard fees and help you understand your contribution. Contact us for a personalised quote.
Why the Support at Home Program matters
The Support at Home program was introduced to fix gaps in the old programs. Many people found the system confusing, with different rules and long wait times.
The new program aims to:
- Create fairer access to funding.

- Provides tailored pathways for short-term, restorative and end-of-life care.
Key features of the Support at Home Program

Expanded funding classifications (8 levels + short-term pathways)
Support at Home classifications are grouped into eight funding levels plus 4 additional classifications for transitioned Home Care Package recipients who have not been reassessed, plus additional short-term pathways for restorative or end-of-life care. This expanded system ensures funding aligns more closely with individual needs, providing greater choice and fairness for clients.

Flexibility & personalisation (quarterly budgets & carry-over rules)
Your yearly care budget will be divided into four quarterly amounts. This makes it easier to see what is available and how it is being spent. If the full amount is not used, you can carry over up to $1,000 or 10 per cent of that quarter’s budget, whichever is higher.

Assistive technology & home modifications (AT-HM scheme)
The new AT-HM Scheme creates a separate funding stream for equipment and home upgrades. Ranging from simple mobility aids to projects like bathroom adjustments, this funding supports independence without reducing your main care budget. Tiers extend from $500 for smaller needs to $15,000 for major works (or higher with evidence for assistive technology).

End-of-life & restorative pathways
Support at Home also includes specific funding pathways for key life stages. Restorative services include allied health therapies, equipment and home changes to help people recover strength and independence. The end-of-life pathway provides up to $25,000 over three months for palliative services, counselling and family support, allowing people to remain at home with dignity and comfort. Access is assessment-based, and while Holdsworth may not deliver all services directly, we provide guidance and referral so families and loved ones feel supported at every step.
How the transition works for existing clients

Holdsworth clients on home care packages
If you were on a Home Care Package (HCP), you transitioned automatically on 1 November 2025. Your funding carried over at an equivalent amount when you transitioned to Support at Home, helping maintain continuity of care. Your annual budget is now issued in four quarterly allocations. Any unspent funds from your previous package remain available to you.

CHSP clients
If you receive support under the Commonwealth Home Support Programme (CHSP), your services will not change until July 2027 at the earliest. If you are a Holdsworth client, we will provide updates and guidance well before that date, so you feel confident about the transition.

No. You will not pay more when you transition to Support at Home, and your funding level has carried over when you transitioned. The mix of services may change after assessment, and we are working with you to keep support consistent.
Funding is split into quarterly allocations, with unspent amounts carried over. There are also eight levels instead of four, giving a better match to needs. Additionally, 10% of your quarterly budget is set aside for care management activities.
You can access a wide range of support, including aged care services, senior care transportation services, senior social activities and senior meal delivery or shopping services. Support at Home includes clinical supports (such as nursing and allied health services), independence support (such as personal care, social support, and transport), and everyday living assistance (such as domestic help, meals, and home maintenance).
