We are an Approved Provider delivering care through Australian government-funded programs, including the Commonwealth Home Support Program (CHSP) and the Support at Home program, which replaced Home Care Packages.
Private and fee-for-service options are also available for people who are not eligible for government funding or prefer to self-fund.

Services we provide
As a trusted community provider, Holdsworth delivers aged care services across Sydney to support people to live safely, comfortably and independently at home. Our Care Partners take the time to understand your goals, preferences and daily routines, so your support continues to reflect your lifestyle as needs change.
We provide flexible support through the Support at Home program, the Commonwealth Home Support Program (CHSP) and other community-based services. In some cases, private or fee-for-service options may also be available. All services are personalised and guided by your assessment and care plan and may include the following:

How to fund your aged care in Sydney
There are several ways to access aged care services, depending on your needs and circumstances. Many people receive support through government-funded programs, while others choose to self-fund services privately.
Holdsworth supports people accessing services through the Support at Home program and the Commonwealth Home Support Program (CHSP). We also offer free aged care at home consultations and provide free My Aged Care navigation support in Sydney, helping you understand the system, your assessment outcome and the options available to you.
People under Home Care Packages (HCP) in Sydney are now supported through the Support at Home program, which replaced HCP.

Eligibility criteria
You may be eligible for government-funded aged care if you need some help with everyday activities and meet the age requirements. In most cases, this includes people who are:
- 65 years or older, or
- 50 years or older as an Aboriginal or Torres Strait Islander person, or
- 50 years or older and experiencing homelessness or at risk of homelessness.
- Assessed as needing support through an independent aged care assessment, such as one completed by the Regional Assessment Service (RAS).

How funding works
Aged care funding is determined by the Australian Government, not by Holdsworth. Your needs are assessed through the Single Assessment System, previously known as ACAT or RAS.
Based on this assessment, the government assigns an individual budget tied to your classification. Budgets are reviewed quarterly, with the ability to carry over 10% or $1,000, whichever is higher, into the next quarter.
You can use your budget across three categories of support:
- Clinical care – fully funded by the government
- Independence supports – you may contribute up to 50%
- Everyday living services – you may contribute up to 80%

Although assessments and budgets are set by the government, you don’t need to navigate the process on your own. Holdsworth can support you as you get started with My Aged Care and help you understand what your classification means and how it applies to your situation. Our team also offers guidance and information to make aged care easier to understand at every stage.

Yes. We provide transportation services, including trips to Prince of Wales Hospital, St Vincent’s Hospital and Bondi Junction medical suites. Return trips can also be arranged.

A trained, local support worker matched to your needs. Holdsworth values connection and community, which is why we aim to send a familiar face whenever possible. This helps you feel comfortable and build a trusting relationship with your support team.
How do I move to Holdsworth from another provider?
You can transition from another provider at any time. Holdsworth can guide you through the process and help ensure services continue with minimal disruption.
