Funding your care in the Eastern Suburbs
Many people in Sydney’s Eastern Suburbs receive home care through government-funded programs. The current system is the Support at Home program, which replaced the former Home Care Packages program in November 2025.
Eligibility for an aged care assessment
You may be eligible for an aged care assessment if you need help with everyday tasks and meet the age requirements. These include:
- being 65 years or older, or
- being 50 years or older as an Aboriginal or Torres Strait Islander person, or
- being 50 years or older and experiencing homelessness or at risk of homelessness.
How funding works
Your needs are assessed through the Single Assessment System (previously ACAT or RAS). This independent assessment determines your Support at Home classification and the level of support you can access.
Based on this assessment, the government assigns an individual budget tied to your classification. Budgets are quarterly, with the ability to carry over 10% or $1,000, whichever is higher, into the next quarter.
You can use your budget across three categories of support:
- Clinical care – fully funded by the government
- Independence supports – you may contribute up to 50%
- Everyday living services – you may contribute up to 80%

Can you help me get to medical appointments?
Yes. Holdsworth provides transportation services for seniors that support travel to local health and medical appointments.
We regularly assist with trips to:
- Prince of Wales Hospital
- St Vincent’s Hospital
- Bondi Junction medical suites
- Local GPs, specialists and allied health providers
We can provide a return trip or work with you to arrange a pick-up time that suits your appointment.
Who will come to my home?
A trained, local support worker matched to your needs. We aim for continuity wherever possible, so you will see familiar faces and feel comfortable with your team. Your Care Partner will also stay in touch with you, your family or your carer to make sure your services continue to reflect your goals and preferences.
How do I transition from another provider?
You can change providers at any time. If you decide to transition to Holdsworth, we will guide you through the steps, including contacting your current provider, checking notice periods and arranging a start date.
Your Care Partner will help you transfer your Support at Home budget and make sure your services continue with as little interruption as possible. Contact us if you’re unsure where to begin.
Do you offer any private or fee-for-service options?
Yes. All services available through the Support at Home program can also be accessed privately. This means you can choose the support you need without waiting for an assessment or if you prefer to pay directly. Private services may include in-home support, personal care, meals, community transport, nursing and other tailored assistance.
