Home Care Package Level 4

Home Care Package Level 4 services are designed to provide a wide mix of supports that help you live safely and comfortably at home. Depending on your assessed needs, it can include:

• Social Activities for Seniors – companionship, group outings and community connection
• Community Transport Services – safe and reliable travel to appointments, shopping or social activities
• Domestic Assistance – help with household tasks such as cleaning and laundry
• Personal Care – support with bathing, grooming, dressing and mobility
• Nursing – in-home nursing care to manage health conditions
• Holdsworth Home Cuisine – Meal Delivery – meal preparation, delivery and grocery shopping support
• Allied Health – services like physiotherapy, podiatry, occupational therapy and more

Your package funds can be used for services that match your care plan and support your independence at home. The funds cannot be used for everyday living expenses such as rent, utilities, holidays, entertainment or medical costs already covered by Medicare or the PBS.

You may qualify for a Level 4 package if you:

• Are aged 65 or over (50+ for Aboriginal and Torres Strait Islander peoples)
• Have more complex or high-level care needs that require regular support at home
• Complete an assessment through the government’s Single Assessment System (previously ACAT or RAS), which confirms your eligibility and assigns your budget

The application process is simple, and you can ask for help at any time. Here are the main steps:

1. Contact My Aged Care – Call 1800 200 422 or visit the My Aged Care website to arrange an assessment.
2. Complete your assessment – The government’s Single Assessment System (previously ACAT or RAS) will confirm your eligibility and assign your funding level.
3. Wait for your package – Once approved, you will be placed in the national queue until your package is assigned.
4. Choose your provider – This is when you decide who will coordinate and deliver your care.

Home Care Package Level 4 funding is made up of a government subsidy plus any income-tested care fee, along with provider management fees. There are a few fees you may need to know about:

• Income-tested care fee – This is set by Services Australia after an income assessment. Full pensioners usually do not pay this fee, but part-pensioners and self-funded retirees may contribute an amount based on their income.
• Care management fee – Covers the ongoing support of a dedicated care manager who works with you and your family to review your care plan, adapt to changing needs and coordinate services.
• Package management fee – Covers the behind-the-scenes work to manage your budget, organise invoices and reporting and make sure your package stays compliant.

Home Care Packages are offered at four levels so that support can be matched to your individual needs:

• Level 1 – Entry-level support for people with basic care needs
• Level 2 – Low-level care for those needing some ongoing help at home
• Level 3 – Intermediate care for people with moderate needs
• Level 4 – High-level care for people with more complex or daily care needs

From November 2025, the Australian Government will introduce the Support at Home program, which will replace Home Care Packages and other aged care programs. The aim is to make services simpler and more flexible.

If you already receive a Home Care Package, your care will continue. When the new program begins, your services will transition smoothly across to Support at Home.
