Effective communication is key to ensuring that your care needs are met and that you maintain a high quality of life. Whether you’re discussing your needs with family members or professional carers, clear and open communication can make a significant difference.

Be clear and specific

When discussing your care needs, it’s important to be as clear and specific as possible. Clearly articulate what kind of support you require, whether it’s assistance with daily activities, medical care, or emotional support. The more specific you are, the easier it will be for your family and carers to understand and meet your needs.

Write down your needs

Sometimes, writing down your care needs can be helpful, especially if you find it challenging to articulate them verbally. A written list can serve as a reference for both you and your carers, ensuring that nothing is overlooked. It also provides a clear document that can be updated as your needs change.

Schedule regular check-ins

Regular check-ins with your family and carers can help ensure that your care plan remains up-to-date and effective. These meetings provide an opportunity to discuss any changes in your needs, address any concerns, and make adjustments as necessary. Consistent communication helps maintain a proactive approach to your care.

If your needs change over time and you are receiving care through Support at Home or the Commonwealth Home Support Program (CHSP), you can request a Support Plan Review through My Aged Care. This review can help you access additional services or a higher level of support tailored to your evolving needs.

A Support Plan Review can be requested when:

Your needs, goals or circumstances change
You need additional services (including access to the Restorative Care Pathway, End-of-Life Pathway or Assistive Technology and Home Modifications)
A time-limited service has ended
