Effective communication is key to ensuring that your care needs are met and that you maintain a high quality of life. Whether you’re discussing your needs with family members or professional carers, clear and open communication can make a significant difference.

Schedule regular check-ins
If your needs change over time and you are receiving care through Support at Home or the Commonwealth Home Support Program (CHSP), you can request a Support Plan Review through My Aged Care. This review can help you access additional services or a higher level of support tailored to your evolving needs.
A Support Plan Review can be requested when:
- Your needs, goals or circumstances change
- You need additional services (including access to the Restorative Care Pathway, End-of-Life Pathway or Assistive Technology and Home Modifications)
- A time-limited service has ended
