If you or a loved one were receiving aged care under the Home Care Packages (HCP) program, from 1 November 2025, you transitioned to Support at Home. Here’s everything you need to know about the changes, what has happened, and how to navigate the new system.

Key benefits of the transition:

Your annual package budget is now divided into four parts – one for each quarter of the year. You can carry over unspent funds, up to $1,000 or 10% of your quarterly budget (whichever is greater).

What are the key changes?

There are several important changes you should be aware of:

1. Quarterly budgeting

Your existing annual budget is now split into quarterly allocations, offering better transparency and financial control. The quarterly funding cycle follows the fiscal quarters:

    • July to September
    • October to December
    • January to March
    • April to June

2. Clearer service inclusions

Under Support at Home, all services will be specified on a service list with clear inclusions and exclusions. This list largely mirrors the former services you accessed through the Home Care Packages Program, ensuring continuity of care.

3. Care management allocation

Care management funding is set at 10% of your ongoing quarterly budgets. This funding is used by your provider to deliver care management support, ensuring you receive the assistance you need to manage your care effectively.

4. Separate funding for equipment and modifications

If you need assistive technology or home modifications, you’ll receive a separate budget through the new AT-HM Scheme – no need to dip into your care funds.

What you need to do

If you transitioned from the HCP Program on 1 November 2025, your services are continuing as normal under Support at Home. Your provider (like Holdsworth) is here to support you with any questions about how the new system works.
