Navigating My Aged Care
Your journey to receiving support through Holdsworth and other service providers begins with the government’s My Aged Care program. My Aged Care provides government funding for you to access services that support your needs and goals, whether it’s getting you to where you want to go, help around your home when you need it, or connecting you with your local community.
What do you need to do?
Step 1
First, call My Aged Care on 1800 200 422. If you’re acting on someone’s behalf, you’ll need to have their permission and Medicare number.Step 2
Your Aged Care will discuss your needs and undertake an assessment.Step 3
Following your assessment, you will be provided information about any funding packages you are eligible for.Step 4
You choose a provider, which may be Holdsworth, who can arrange services and help manage your My Aged Care package following your referral.
The Commonwealth Home Support Program (CHSP) provides funding for support services to assist people over 65 (or over 50 for Aboriginal and Torres Strait Islander people) to remain living at home and active in the community.
CHSP services include activities that encourage independence and connection to your community based on your goals.
CHSP is not designed for people with more complex care needs. If you have higher needs, you may consider a Home Care Package to make sure you get the right support.
A Home Care Package is an allocation of funds provided by the government to purchase products and services that help you stay at home and live a quality, safe and independent life.
You need to have an assessment to decide what level of package. There are four levels depending on your care needs. Once you have a package, it stays in place for the long term. If things change, you can have another assessment so you can access more services.

As an approved aged care provider we give you support and advice to make the most out of your funding, whether that’s through the Commonwealth Home Support Program or through a Home Care Package.

The Charter of Aged Care Rights is easy to read and focuses on 14 high-level consumer rights. The new Charter makes it easier for consumers, their families, carers and representatives to understand what they can expect from an aged care service.
It is Holdsworth’s responsibility to ensure you have:
- Received a copy of the new Charter of Aged Care Rights, signed by Holdsworth
- Understood the Charter
- Had the opportunity to sign a copy of the Charter

The Aged Care Quality Standards apply to all services and supports that Holdsworth provides.
The Aged Care Quality Standards focus on quality outcomes for consumers. This makes it easier for consumers, their families, carers and representatives to understand what they can expect from a service. It also makes regulation simpler for providers working across multiple aged care services, and encourage innovation, excellence and continuous improvement.
