NDIS Support Coordinator

Part-time (approx. 20 hours/week)
Fixed Term (6 months)
SCHADS Award: Level 3 Social and Community Services

About the Role

We are seeking a Support Coordinator on a Part Time basis for 3 to 4 days per week, with two of these being Monday and Tuesday (preferably).


In this role you will:

  • Assist NDIS participants to navigate the NDIS System, advocating for their
    individual needs.
  • Promote person centred outcomes for participants to support their individual aspirations and identified goals.
  • Develop individualised support plans.
  • Support Participants in implementing their NDIS Plan to maximise the value for money through their supports.
  • Monitor and assess progress toward goals, regularly updating reports and sharing updates with Participants and their Families.
  • Support Participants to connect and engage with providers, to access supports, and build Participant capacity to maintain these relationships.
  •  Communicate, engage and collaborate with Participants and their families, service providers, NDIS Planners and other community health and service providers.

Requirements

We are looking for a candidate available 3-4 days per week with two of these days being Monday and Tuesday, preferably.


We are looking for a candidate who can be flexible according to client requirements.


To be successful in this role, you will need to have knowledge of the NDIS Scheme and processes.

Previous experience as a Support Coordinator is desirable, however those with knowledge of the NDIS and transferable skills will also be considered.

You will also need to hold, or be willing to obtain, the following:

  • Police Check
  • Working with Children’s check
  • NDIS Worker Screening Clearance
