Are you waiting for a Home Care Package? Are you looking for clinical and allied health support and advice in a safe and welcoming environment? Our Holdsworth Wellness Hub is a free, holistic clinical and allied health support service for Woollahra residents who are waiting for a Home Care Package.

Holdsworth Wellness Hub

The program is designed to meet your individual needs and is flexible to suit your lifestyle and choices. Beginning at Gaden Community Centre, we will transport you to and from your home to meet with our registered nurse, plus a morning of discussion and fun, including:

  • Transport to and from your home
  • Assessments and support to access clinical and allied health services
  • Support to navigate My Aged Care
  • Light exercise 
  • Morning tea and refreshments

Following your session at Gaden, we will put in place tailored services to meet the needs discussed at a time and place that suits you, and might include an exercise program or allied health treatments, physio, wound assessments, community transport or home cuisine delivery. Services run for free for approximately 12 weeks, during this time we will work with you to access services ongoing.
