About the role
We are currently looking for an experienced Community Registered Nurse to join our team. We need someone who can work independently and efficiently to supported older community members, including people living with dementia. The role is supported by our Clinical Excellence Leader, and you are part of a collaborative team of Care Coordinators and support workers.
Key responsibilities will include:
- Supporting the clinical needs of clients in community/in-home environments in line with care plans and best practice (e.g., wound care, diabetic support, medication management).
- Conducting clinical assessments and contributing to the development of care plans.
- Providing training for support workers to ensure compliance with the new Aged Care Act.
- Monitoring and ordering clinical supplies to ensure seamless care delivery.
- Liaising with primary and allied health providers to achieve optimal client outcomes.
- Working with our Intake and Engagement Officer and referral partners to support effective clinical transition for new clients or those experiencing discharge from hospital.
- Participating in internal reviews and continuous improvement processes to maintain the highest standards of clinical care.
About you
We are seeking a candidate open to either part time or full-time work, with a minimum 25 hours spread across Monday to Friday and:
- Holds full, unrestricted AHPRA registration as a Registered Nurse.
- Has a minimum of 4 years of general nursing experience post-registration. Experience working in a community setting would be highly regarded.
- Demonstrates excellent communication skills, particularly in supporting elderly clients and working with families.
- Has a strong understanding of community nursing and the in-home care environment, including the Aged Care reforms and strengthened Quality Standards.
- Holds a valid driver’s license and has access to a vehicle.
- Has current First Aid/CPR certifications (or holds them through their registration).
- Can provide a National Police Check upon request.
